Introduction
The legendary activity from BFA is making a comeback in the new patch 11.1.5. Horrific Visions are back on the menu. With some new twists and changes, they will keep you occupied for the remaining part of the season until we get 11.2. This part of the update was released on 20th May on NA and 21st May on EU. Let’s run you through all the things that you need to know and be ready to dive into this incredible piece of content fully prepared.
What is new in Horrific Visions?
Here are some new changes added to the unforgettable Visions of N’Zoth. Relatively, the area you’ll explore remains the same, but with additional twists and new ways to play it:
- You will not need the keys anymore, you can freely enter as many times as you want
- The legendary cloak, Ashjra’kamas Shroud of Resolve, is not required or used in this new addition of the mode
- Soridormi is a new companion, similar to Brann - She can play the role of the Tank, DPS, or Healer, and is not affected by Sanity
- There are now 3 additional masks, meaning you can challenge up to 8 Masks at once.
- Item level has now been updated to match the Season 2 one, and you can obtain Hero Track gear from 5-Mask runs when you get all objectives done
There are a bunch of new rewards for players to unlock on top of all the old ones available as well.
Where to find the new Horrific Visions?
The new Horrific Visions are located in Dornogal in Khaz’Algar. You can find them on your Map, as they will be marked with the exact location within the city. You can also find the Hourglass of Horrific Visions, which is a brand-new talent tree that you can use for this new activity. You can launch and start the runs from Soridormi, which is also located within this hub. Once you activate the portal with Soridormi, you will enter the big Safe Room, where you can do all the things regarding the Horrific Visions, like another hub.
How does Hourglass of Horrific Visions work?
This new talent tree will help you get some additional powers and strengths to help you clear Horrific Visions far more efficiently. Though it is a bit time-gated, it will still give you a bit of help to feel stronger inside the instance. Here are all the things you need to know regarding it:
It’s a brand new talent tree that helps you clear more sanity as you unlock more upgrades
You will be able to unlock a limited number of permanent upgrades each week, progressing the bar and getting new upgrades.
Each upgrade costs an increasing number of Displaced Corrupted Mementos, which you can exchange for 1 Echo of N’Zoth from Augermu.
What can you do within the Horrific Visions Safe Room?
Once you enter the room, you will see the familiar location that was present during the original run of this event. The titan room will be filled with all sorts of things you can interact with and check for things to prepare for your upcoming runs. Here are all the things you need to know:
- You can set up your companion’s role (DPS/Tank/Healer). Interact with the Bronze Hourglass to recruit Soridormi.
- Activate the Masks for your run, We’ll explain everything you need to know regarding the masks in this guide.
- You can check your Tribute progress as well here.
- You can freely leave the Horrific Visions by interacting with the portal within the room.
Main objective within the Horrific Visions and Sanity Bar
There is not much complicated mechanics within it. The most important thing you need to pay attention to is your Sanity meter; keep it as high as you can. After that, you will need to defeat Thrall if it’s Orgrimmar or Alleria in case you’re doing the Stormwind version of the Horrific Visions of N’Zoth.
Sanity bar is your most important thing, as previously mentioned. It will be a purple bar located on your screen that slowly charges as you progress and fight the mobs within the instance. If the bar hits 0, you will immediately be teleported outside the run, and it will end there, so making sure you prevent that from happening is your key to victory. The best tip is to avoid getting hit by abilities, as they will drain your bar way faster. Of course, some abilities might be unavoidable, but making sure not to get hit by those that can be dodged is important.
Sanity is increased by using Sanity Restoration Orbs or by killing Elites within the cities, in case if you have it unlocked in your talents. A lot of talents will assist you in getting more Sanity or losing less Sanity within the instance, so you will not have to worry much once you have all those unlocked. Since you’re limited to how much you can unlock each week, you can read up on all the talents once you’re picking them and adapt your strategy or take the ones that you feel will be the most useful to your personal experience.
Other things you need to know are that each Vision is divided into multiple subzones, each has its own level of difficulty and challenges awaiting within it. There are a lot of things to do within these subzones, with some side objectives and activities awaiting you, and to finalize the subzone, you will have a miniboss ready to be challenged and stopped by you. Once you fully clear the subzone, you will get a teleporter, so you will be able to the starting area of the vision. Each subzone has different tiers, which are pretty much like the difficulty of the activity. Here are all the tiers listed and explained:
- Tainted Zone (Tier 1): These zones are the most basic ones, and the Sanity level here is not drained fast. You lose around 6 Sanity per second.
- Corrupted Zone (Tier 2): The medium one in difficulty and Sanity drain, you lose approximately 8 per second.
- Lost Zones (Tier 3): These are the ones that are the most challenging. You lose around 10 Sanity per second within it.
Within these zones, there are also madnesses, which are harder and more taxing to your experience, with a bunch of sanity-draining mechanics to hallucination, and debuffs that await you each depending on which zone you’re in. The current active madness will be displayed on top of your screen, so you will know exactly what it does.
The run can end in the following ways:
- You die, and there is no one to resurrect you. (There is also a perk that allows you to resurrect once, but you need to have it unlocked first via Hourglass.)
- Once your Sanity drops to 0, you will be teleported instantly from the instance, and the run will end, meaning this is another way to lose this battle.
- You manage to defeat the last boss within the given city you’re in (Thrall or Alleria) and emerge victorious.
In the first few weeks, you will not be able to clear the full activity except for the few side zones. This is due to time-gated upgrades that are not immediately available to be unlocked. You can collect Displaced Corrupted Mementos to upgrade as much as you have available to make these runs last longer, so you can make some progress.
Horrific Visions Potions
Some potions are located within the Stormwind and Orgrimmar Horrific Visions, each one does something different, however, you can’t drink all the potions. Each color changes depending on the run. To recognize which potion you can’t drink, you will need to go to a certain location within the cities and find clues there.
- Stormwind - Within Stormwind, you will need to go to the Cathedral Square right after you enter it. At the end of the first street is a pack of enemies with Fallen Voidspeaker and Fallen Heartpiercer. You can find the corpse of Morgan Pestle with a potion next to him. The color of that potion is the bad potion you can’t use in this specific run, so make sure to memorize that.
- Orgrimmar - Here, you will also need to find specific clues within the starting area, go left from Thrall’s Boss room, and to the second building. There you will find a Voidbound Ravager. Finding a potion there will be your bad potion for the run.
The following potions are available within the Horrific Visions:
- Potion that decreases your Sanity by 100
- Potion that increases your Sanity by 100
- Sickening Potion Buff - Take 5 % less damage from ALL sources. On expiration or removal, you will start vomiting.
- Spicy Potion Buff - You will start breathing fire and deal damage to the surrounding enemies. On expiration or removal, you’ll burn and receive damage over time.
- Sluggish Potion Buff - Heals you for 2 % of your maximum HP every 5 seconds. On expiration or removal, you will be slowed for a short duration.

All Masks in the Horrific Visions, and what do they do?
There’s a total of 8 Masks available within this rendition of Horrific Visions, each one has a special effect, and once you have all 8 active, you can get a special bragging rights achievement for it, The Mad. With 5 Active, you can also get Hero Track gear. Here’s all the masts and what they do:

What are Tributes within the Horrific Visions?
Tributes are bonuses that are automatically unlocked by running Horrific Visions. There is a total of 4 Tributies, and each one of them has a tier. Here’s all of them listed:
Tribute of Clear Sight
To get this one, you need to collect potions and crystals, and to empower it further, get even more of the same thing.
- Clear Sight Rank 1: In the Horrific Visions, you will be able to see and loot chests within the Tainted areas.
- Clear Sight Rank 2: In the Horrific Visions, you will be able to see and loot chests within the Tainted and Corrupted areas.
- Clear Sight Rank 3: In the Horrific Visions, you will be able to see and loot chests within the Tainted, Corrupted, and Lost areas.
Tribute of the Experimatal Destabilization
This one is unlocked by killing bosses and further progressed by adding more boss kills to your kill count.
- Experimental Destabilization Rank 1: While above 50 % of your Sanity, you deal 4 % more damage, OR while under 50 % of your Sanity receive 4 % reduced damage.
- Experimental Destabilization Rank 2: While above 50 % of your Sanity, you deal 8 % more damage, OR while under 50 % of your Sanity receive 8 % reduced damage.
- Experimental Destabilization Rank 3: While above 50 % of your Sanity, you deal 12 % more damage, OR while under 50 % of your Sanity receive 12 % reduced damage.
Tribute of the Vision Hunter
Unlocked by killing enemies within N’Zoth’s Visions, further empowered by progressing and killing even more enemies.
- Vision Hunter Rank 1: In Horrific Visions, after killing an enemy, you get 1% increased Movement Speed and Haste for 60 seconds, this can stack up to 5.
- Vision Hunter Rank 2: In Horrific Visions, after killing an enemy, you get 2% increased Movement Speed and Haste for 60 seconds, this can stack up to 5.
- Vision Hunter Rank 3: In Horrific Visions, after killing an enemy, you get 3% increased Movement Speed and Haste for 60 seconds, this can stack up to 5.
Tribute of the Elite Extermination
Unlocked after killing Elite enemies within Horrific Visions and empowered by killing more of the big baddies.
- Elite Extermination Rank 1: In Horrific Visions, after defeating an Elite enemy, restore 50 Sanity.
- Elite Extermination Rank 2: In Horrific Visions, after defeating an Elite enemy, restore 100 Sanity.
- Elite Extermination Rank 3: In Horrific Visions, after defeating an Elite enemy, restore 150 Sanity.
Helmet Corrupted Enchants
BFA used to be remembered by an interesting mechanic that was called corruption. We’re getting something quite similar. till the end of Season 2, we will be able to use a brand new corruption effect and pick which one we want to apply to our helmet. There are Rare and Epic enchantments available for players, and here’s what they do:
Rare Helmet Enchants
- Lesser Rune of Twilight Devastation: This one allows your attacks to have a chance to trigger a beam of Twilight Devastation, dealing up to 2535585 Shadow damage to enemies in front of you.
- Lesser Rune of the Void Ritual: You gain the Void Ritual, giving your spells and abilities a chance to increase all secondary stats by 47 every second for 20 seconds.
- Lesser Rune of the Twisted Appendage: Your attacks have a chance to spawn a tentacle that Mind Flays your target for 95085 Shadow damage every second for 10 seconds.
- Lesser Rune of Echoing Void: All your damaging abilities build the Echoing Void. Each time it builds, Echoing Void has a chance to collapse, dealing 71842 Shadow damage to all nearby enemies every second until no stacks remain.
- Lesser Rune of Gushing Wound: All your damaging spells and abilities have a chance to cause your target to ooze blood for 264418 damage over 7 seconds.
- Lesser Rune of Infinite Stars: All your spells and abilities have a chance to strike a nearby enemy with an Infinite Star, dealing 211298 Arcane damage and increasing their damage taken from your Infinite Stars by 73954, stacking up to 6 times before resetting.
To unlock these rare enchants, you will need to complete the Borrowing Corruptions quest chain. In this quest line, you will need to collect 8 Black Blood Residue and 500 Displaced Corrupted Mementos within the Horrific Visions. This quest can be found in the Horrific Visions hub area in Dornogal.
Epic Helmet Enchants
- Greater Rune of Twilight Devastation: It allows your attacks to have a chance to trigger a beam of Twilight Devastation, dealing up to 4346700 Shadow damage to enemies in front of you.
- Greater Rune of the Void Ritual: You gain Void Ritual, giving your spells and abilities a chance to increase all secondary stats by 82 every second for 20 seconds.
- Greater Rune of the Twisted Appendage: All your attacks have a chance to spawn a tentacle that Mind Flays your target for 163002 Shadow damage every second for 10 sec.
- Greater Rune of Echoing Void: All your damaging abilities build the Echoing Void. Each time it builds, Echoing Void has a chance to collapse, dealing 123157 Shadow damage to all nearby enemies every second until no stacks remain.
- Greater Rune of Gushing Wound: All your damaging spells and abilities have a chance to cause your target to ooze blood for 453292 damage over 7 seconds.
- Greater Rune of Infinite Stars: All your spells and abilities have a chance to strike a nearby enemy with an Infinite Star, dealing 211298 Arcane damage and increasing their damage taken from your Infinite Stars by 73954, stacking up to 6 times before resetting.
After you complete the first quest line, you will get another quest, Enhancing Corruptions, which will help you get your old Rare enchanments upgraded to Epic quality and make them far stronger. You will need to get 8 Black Blood Coagulate and 1000 Displaced Corrupted Mementos.
Horrific Visions Gear and Cosmetic Rewards
Horrific Visions of N’Zoth are all filled with loot and interesting rewards for us players to obtain. From the bunch of catch-up gear to unlock, to gear up your alts or add up some for your main in the slots you might miss, there are plenty of things to unlock. Even if you don’t have gear, you can obtain a bunch of unique and fun cosmetics from mounts and pets to transmogs.
There is also a specific loot system set for this content, due to all the different factors that can affect your run with all the different masks and subzones, completing each thing will, of course, give you better rewards. The following cosmetic rewards are obtained by completing certain activities in the Horrific Visions, and they can be directly earned as drops or by completing certain things within the instance:
- Felreaver Deathcycle to get this one, you’re required to complete a puzzle involving both Horrific Visions. You must first put on at least 1 Mask and head to the Dwarven District in the Horrific Version of Stormwind. After you reach the destination, you will find a Voidfire Deathycycle near the Auction House. Defeat it to send it to Dornogal, where it will further require different spare parts scattered across both Horrific Visions, and once you get them, you can assemble it.
